For those arriving in the UK from 8 June onwards, journey and contact details must be provided prior to travel. You cannot submit the form until 48 hours before you are due to arrive in the UK.
You will need to show your completed form when you arrive at the UK border, either by printing a copy or showing it on your phone.
The government will use this information to contact you if you or someone you have travelled with develops coronavirus (COVID-19) symptoms, and to check that you are self-isolating for the first 14 days after you arrive in the UK.
